Overview:
We are looking for a Case Manager PC to join our team. This position will directly report to the Branch Director PC or Assistant Branch Director PC and is responsible for working collaboratively to assess, plan, facilitate and advocate for the options and services to meet a patient’s health needs.
Coordinates with the Case Manager Supervisor and with DADS to complete the service plan authorization process
Coordinates all aspects of service delivery, ensuring that all services are delivered according to the service plan
Completes on-going monitoring with the client. The client is monitored for appropriateness of services, for health and safety issues, for re-evaluation of the service plan
About You:
High school diploma or equivalent; some college preferred
One year paid professional experience with the aging or disabled population
This role requires full COVID-19 vaccination.
