The Certified Diabetes Educator-RN reflects the mission, vision, and values of HealthNet, adheres to the organization’s Code of Ethics and Corporate Compliance, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards. The Certified Diabetes Educator (RN) is accountable and responsible for the provision of professional nursing, consultation, and educational services for persons with diabetes, and provides community health education. The Certified Diabetes Educator (RN) develops curriculum on health, including research and production and act as a health care consultant by providing information on the management of diabetes for patients of all ages from infancy to geriatrics. The clinical nurse participates in decision-making, teaching, mentoring, and collaboration with the multidisciplinary team to advance the patient plan of care, achieve outcomes and enhance operational efficiency. The nurse adheres to the ANA Scope and Standards of Professional Nursing and the Code of Ethics for Nurses with Interpretive Statements.
Possesses clinical expertise:
Employs a combination of education, experience and environmental awareness that provides high quality, person-centered care to patients.
- Provides education related to diabetes and the disease process; medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injections.
- Develops and implements educational programs for diabetic staff, patients, and families
- Advises patients as to long-term complications
- Provides clinical resources and consultation to staff and providers
- Assist patients in monitoring blood glucose for the purpose of medication control and adjustment for both insulin and noninsulin dependent diabetics
- Assess, monitor and implement treatment plans for patients at various stages of the disease. The focus is on prevention of complications and management of stable diabetes
- Provides ongoing assessment and recommendations to the treatment team and patient/family for adjustments in the plan of care as required
- Document assessments, interventions and plans including recommendations and goals, progress notes and discharge summaries
- Coordinate community outreach education classes
- Coordinates patient care with HealthNet’s Chronic Care Management Program, RD, Health Education Program, Clinical Pharmacy Program, Providers, and Care Team Members.

Advances clinical expertise from novice to expert through pursuit of formal and informal educational opportunities.

Requires an Associates of Nursing (ASN); Bachelor of Nursing (BSN) preferred.
Requires that the RN has graduated from a nationally accredited nursing program.
Requires current state of Indiana license as Registered Nurse.
Requires Certified Diabetes Educator (CDE)
Requires Basic Life Support certification through the AHA.
A minimum of two years’ experience in healthcare setting, preferably in diabetes prevention or health education program.
May require travel between health centers.
EVALUATION OF PERFORMANCE:
The Certified Diabetes Educator (RN will receive annual performance evaluations that rate job performance in accordance with HealthNet vision, mission, policies and procedures. The evaluations may include, but not be limited to, performance of many of the specific skills and abilities noted above. The Certified Diabetes Educator (RN) is expected to present an exemplary professional image; maintain a positive attitude toward work and HealthNet; display a willingness to accept and complete assigned job responsibilities and duties; demonstrate the ability to independently exercise judgment, supervise staff if applicable, and complete all phases of assigned tasks or projects; and serve as a positive role model for other HealthNet staff.
