The incumbent works in collaboration with the Primary Care Provider (PCP), Care Manager Registered Nurse (RN) and the Medical Support Assistant (MSA). Together, with the patient and their caregiver support system, they work to manage the care of a panel of adult patients across the continuum. The incumbent possesses the necessary skills and knowledge to care for patients utilizing novice or basic knowledge of practical nursing skills and procedures, and functions as a member of the PACT by participating in planning, implementation and evaluation of nursing care using a focused assessment. She/he competently cares for the adult patient with a variety of physical, behavioral and/or social problems, administers medications via all routes, except intravenous, and carries out basic treatments as ordered, monitors patients’ response to medication and treatments, reports changes and documents. The incumbent recognizes changes in patient’s conditions and reports changes to either the RN or PCP; participates in providing or reinforcing patient education to both patient and their family in relation to health and wellness promotion, chronic disease prevention or management, medications and treatments, procedures, equipment and devices or any education deemed necessary and documents the effectiveness of the education. She/he performs escort and sitter duties based upon the care needs of the Veteran and must have functional knowledge of Reusable Medical Equipment (RME) as well as which reusable instruments, equipment, medical devices, or supplies are used (inventory) in the ambulatory setting. The incumbent’s primary responsibility will be daily management of the PCP’s scheduled appointments which is accomplished through assisting the PCP with preventative care and/or chronic disease management by obtaining vital signs, offering patient education, completing screening questions for a variety of preventative and/or chronic disease measures, managing immunizations, injections and all other ordered medications, and assisting with exams/procedures (pelvic exams, joint injection, wound care, Teleretinal, Telederm, EKG’s, Phlebotomy, bladder scans). The incumbent’s other responsibilities include but are not limited to the following: Assisting the PACT team or other Interdisciplinary team members with panel management such as follow-up of lab and test results, patient education and care coordination, and being an active participant in the planning, implementation, and evaluation of care.Participating in daily Huddles and team meetings with PACT team members.Demonstrating, through interactions, the ability to communicate effectively and professionally with both internal and external customers.Maintaining knowledge of and properly initiating nurse protocols and Standard Operating Procedures (SOPs) to help deliver holistic care; conferring with LPN Team Leader (TL), Care Manager, PCP, or appropriate person to deliver care when needed.Using motivational interviewing and health coaching to guide patients in achieving goals of health promotion and disease prevention.Participating in telephone work and viewing alerts by; completing phone prevention up to 48 hours before scheduled appointment, completing reminder calls and follow-up calls as delegated by the RN or PCP, assisting with completion of Secure Messaging, completing view alerts assigned within 48 hours, providing New Patient Orientation when encountering new patients.Checking daily quality control checks on testing and other equipment/supplies and restocking as needed. Informing the Charge Nurse (CN), LPN Team Leader or Nursing Leadership of broken equipment needing repair.Providing Patient Home monitoring equipment teaching as delegated by Provider or RN Care Manager.Recognizing and appropriately responding to breakage/malfunction or loss of equipment, safety hazards or supply deficiencies and reporting to appropriate personnel for corrective action.All other duties and responsibilities as assigned by RN or Nurse Manager on duty.
