PA School Psychologist certification required.

– complete online and print your results immediately – effective 12/31/14, submit electronically – register & pay online, then go to the fingerprint location of your choice to be printed – includes physical & tb test from within a year – Arrest/Conviction Report

– A separate form (Act 168) must be completed for:

2. All former employers that were school entities; and

3. All former employers where you were in a position that involved direct contact with children.
