Acts is currently seeking qualified candidates for the role of Social Services Coordinator in our skilled nursing neighborhood.

The Social Services Coordinator provides residents and their families with medically related social service intervention. Participates in the admission process with the assistance and consultation of the Nursing Home Administrator and Director of Nursing. Assesses the psycho social needs of residents and provides supportive services to residents and families, as needed. ND270 Granite Farms Estates

Job Requirements

The ideal candidate will meet the following requirements:

  • Bachelor’s degree in social services or related field
  • Minimum of two years’ experience in a long-term care setting
  • General knowledge of social services required in medical/health settings
